Check In Documents for Wilderness Canoe Trips

Every Crew Advisor wants their Crews Check In to go quickly and smoothly.  Listed below is all of the paper work that you need to have completed PRIOR to arrival at a Northern Tier base.  If you have a question on what information is needed to complete a form, please give us a call.  We will be more than happy to help you complete the documents.

Northern Tier Class 3 Medical Form This is the ONLY Medical/Health Form accepted by Northern Tier for Wilderness Canoe Trips.  Adults and Youth MUST have this completed and signed by a Physician prior to arrival.
National Tour Permit All Crews must have a National Tour Permit completed and approved by their local Council and the Regional Office.  Unlike most instances when the type of Tour Permit is determined by distance, Units attending all of the High Adventure Bases, must have a National Tour Permit on file.  This is a KEY document in planning your trip.
Swim Classification Form Leaders and participants must complete a swim test prior to arrival at a Northern Tier base.  Each leader and participant, must be rated a "Swimmer" using the BSA Swim Test.

Participant Policies All Scouts and Venturers must sign off that they have read and understand the participant policies of Northern Tier.

Crew Roster Please have this completed prior to arrival.
Diversity Survey Please take a moment to complete.  This Survey helps us determine who we are serving.
